In order to obtain the emergency identification card for expatriates and tourists, interested citizens can download the form free of charge from the partner sites (GNR, PSP, INEM and Associação Safe Communities Portugal) and complete the available document. You can download it by clicking here.

In addition to the name and age, the requested data goes to the primary residence address (where the person normally resides and not when traveling), the telephone with an international dialing code of police of this address, the place of stay in Portugal and the name and telephone number of any person to contact in case of emergency. You must also provide data on your insurer (if applicable), as well as a description of diseases, allergies and medications that concern you.
